A recent report by a major classification society stated that a spate of lost anchors and chains was down to poor maintenance and inspection. They also state that corrosion of the brake system with worsening environmental factors was the primary cause. Another cause of lost anchors is a poor inspection regime that fails to find loose components in the cable system that subsequently come out, detaching the anchor.
