Violations and mistakes can often be prevented if there is a heightened awareness of what needs to be done and why to ensure safe operation. This module has been prepared to inform or remind those on board of the requirements in place under the International Safety Management (ISM) Code, how company procedures work to protect the vessel and those on board through safe operations and how a positive safety culture can help avoid active failures.

Despite companies having protective measures in place for the safe running of their fleet, there are often violations and mistakes that are latent and do not result in accidents. However,  history has shown that these same  violations and mistakes can result in serious accidents affecting the vessel and its cargo, the crew and others, the environment, the reputation of the company, and future vessel employment.
